You have insurance. Your card is in your wallet. And when your surgeon's office calls the insurer for pre-authorization, it comes back approved. You feel covered — until you see the bill.

The average HDHP deductible in 2026 is $1,600 for an individual and $3,300 for a family. The average out-of-pocket maximum is $7,000 for an individual and $14,000 for a family. If you need a $30,000 knee replacement and your out-of-pocket max is $7,000, you pay $7,000 before insurance covers anything — and that is a best-case scenario. If any provider involved in your surgery is out-of-network (the anesthesiologist, the assistant surgeon, the facility), surprise billing protections help but the out-of-pocket exposure can still climb.

The math nobody does before surgery

Your HDHP out-of-pocket max of $7,000 sounds better than $30,000 cash-pay. But compare it to $8,500 to $12,000 all-in for the same surgery in Colombia. The difference is $1,500 to $5,000 — and in Colombia, you get the surgeon of your choosing, a recovery house, and zero fighting with insurance over claims for the next 6 months.

For the complete cost comparison, see US Cash-Pay vs Colombia. For how to use HSA funds toward either option, see HSA and FSA guide.

When your deductible resets

If you are approaching the end of the calendar year and have already met a portion of your deductible, scheduling surgery before January 1 means you pay less out-of-pocket. If your deductible resets on January 1, a December surgery uses this year's progress; a January surgery starts from zero.

Frequently asked questions

Is surgery abroad covered by my HDHP?

Generally no. HDHPs cover in-network domestic providers. Surgery abroad is an out-of-pocket expense — but it may be eligible for HSA/FSA reimbursement as a qualified medical expense.

Can I negotiate my out-of-pocket cost with the hospital?

Yes. Many hospitals offer prompt-pay discounts (10 to 40 percent) on the patient-responsibility portion. Ask the billing department before your surgery date. See self-pay negotiation guide.
