The playbook

Before you drop coverage, in this order

None of these is a trick. They're the steps most people skip when the renewal notice lands.

01

Check Medicaid first

If your income dropped, you may qualify. Check at HealthCare.gov or with your state Medicaid agency before you pay a higher premium.

02

Compare the whole year

Premium plus deductible plus coinsurance is what you'll actually pay. A cheaper premium with a much higher deductible can cost more if you need a procedure.

03

Use sliding-fee care

Federally funded health centers adjust fees to your income and family size (HRSA). Find one near you.

04

Get the cash price in writing

For anything planned, ask for the written self-pay price or a Good Faith Estimate before you book. Then work the playbook.

05

Price it abroad too

When the US cash price is still out of reach, compare a written, itemized quote from a private clinic in Medellín.

These are prices clinics and practices in Medellín publish themselves, with the source and date for each at the bottom of this page. Anything not on the list needs a written quote based on your records.

Before you compare, check how much of your deductible is left this year. If you've already met it, staying home may be cheaper. Here's how to do that math.

Questions

Why did my premium go up so much?

The enhanced ACA tax credits expired after 2025. KFF found enrollees' average premium payments rose 114% (KFF).

Should I drop my coverage?

That's your call, but know the trade-off: you can usually only enroll during Open Enrollment or after a qualifying life event. Drop coverage and an emergency in between is all yours to pay.

Does care I pay for abroad count toward my deductible?

Ask your plan. Don't assume it does.
